Summary

A 59-year-old patient reported pain on the right side of the neck and a new weakness in the arm dorsiflexion on the right.
A tumor of the right paravertebral cervical soft tissue was found, histologically a malignant peripheral nerve sheath tumor (MPNST). Thirteen years earlier, a squamous cell carcinoma of the right tonsil had been surgically removed and a cystic squamous cell soft tissue tumor in the area of ​​the ipsilateral cervical soft tissue had been radiotherapy.
A high paraplegic syndrome developed with phrenic nerve palsy. Palliative chemotherapy did not improve. The patient died three months later.
MPNST are rare and can occur as a late sequel to radiotherapy. In former radiation fields, radiation-induced secondary neoplasia should therefore be included in the differential diagnosis.
